Family Case Management

The Alisal Community Healthy Start Family Resource Center provides intensive case-management services using the Strengths Based Model.  The families are referred by the Monterey County Department of Social and Employement Services, Family and Children's Services social workers.  The emphasis of the program is to provide prevention and intervention services to families who are involved in the Child Welfare System.  The Case Manager works closely with the DSES social worker and coordinates services that will improve the well-being of the family and prevent future involvement with Child Protective Services.  Services include home visits, family assessment, monitoring and development of a case plan, information and referral services, and a monthly family suppport/educational group.  Services are linguistically and culturally appropriate.
